• Copy of Payslip (with company details and contact nos.) for the last 3 months

• Latest Income Tax Return/W2 Form

     

      A.2 Additional if Sole Proprietorship (all of the ff)

• Business permits and licenses for the current year

• Photocopy of Certification of Registration issued by DTI

• Photocopy of Business ITR/Audited FS for the past 2 years

• Copy of Lease Contact and Title for rental/lease income

      A.3 Additional for OFWs (either of the following)

• Proof of monthly remittances

      A.4 Additional if Transacting through an Attorney-In-Fact (all of the ff)

• Two (2) original copies notarized/consularized Special Power of Attorney
